Roth Conversion Fees: (Current as of 02/10/2010)

Please contact each provider directly for the most current applicaple fees.

LSX Valuation Report:  $250 for up to 5 policies, plus $50 per policy in excess of 5.

Provident Group:  $95

IRA Plus Southwest, LLC:  $75 Conversion Fee or $100 Re-Registration Fee

Dunnam & Dunnam, L.L.P. (escrow agent):  $100

UW Trust Company (escrow agent):  $250

Life Partners, Inc.:  None
